We would like to hear about your experience as a parent carer of the Education, Health and Care Plan (EHCP) process, from the moment your made your first enquiry to the final plan. We call this “Journey Mapping“. Please include as much information as possible as we will use your feedback to improve the service where your experience was not good or to share good practice where you had a positive outcome. Your feedback will be anonymous so do not include any names that might be identifiable. As a thank you for your time, we will send you a £5 Tesco voucher.
